If your spouse, domestic partner, child or other family member is enrolled in a UC-sponsored health plan (medical, dental or vision),
you should have received a packet in the mail asking you to provide documentation that verifies their eligibility for coverage.
All faculty, staff and retirees who have one or more family members enrolled for coverage must provide the requested documentation.
The deadline for submitting documentation is May 17, 2012. If you do not respond, you run the risk of having yourself and your family
members de-enrolled from plan coverage.
